Whether you need a personal copy or are transferring care, start by identifying the records and destination, then follow the office’s release process.

1. Identify What You Need

Decide whether you need a personal copy, selected records or a transfer to another healthcare provider.

2. Provide the Destination

Have the receiving provider’s name and contact information ready when records need to be sent elsewhere.

3. Complete the Request

Follow the office’s records-release steps and provide any authorization needed to process the request.
